1. To create your account go to formed.org. (or click here)

2. Click on "Sign Up"

3. Click on "Sign Up as a Parishioner"

4. Type "St. Sylvester" in the parish box

5. Our parish will be at the top of the list.

6. Click "next"

7. Create your account.

8. Enjoy.
